29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. a) 2089 Terrier Ave, Building 566C, Dam Neck Annex, Virginia Beach, VA 23461: On or about October 28, 2014, and times prior thereto, the foreman and an employee operated a portable table saw and a skill saw to cut wood without eye protection. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: a) 2089 Terrier Ave, Building 566C, Dam Neck Annex, Virginia Beach, VA 23461: On or about October 28, 2014, and times prior thereto, the circular saw blade on the Rigid TS2400-1 portable table saw was not guarded to enclose the approximately 1-5/8" of exposed blade. No abatement certification or documentation required.
