29 CFR 1926.451(b)(4): Each end of a platform, not cleated or otherwise restrained by hooks or equivalent means, did not extend over the centerline of its support at least 6 inches (15 cm):    On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA; employees observed scaling the frame of the scaffold were exposed to fall hazards where scaffold boards were estimated to not extend the minimum six inches beyond the centerline of the support.    a. 7 feet to the lower level where Greenway employees were operating at the rear, left of the building.

29 CFR 1926.451(b)(5)(i): The end of a platform 10 feet or less in length extended over its support more than 12 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees and/or materials without tipping, or has guardrails which block employee access to the cantilevered end:  On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA; employees observed scaling the frame of the scaffold were exposed to fall hazards where scaffold boards shorter than 10 feet were estimated to extending beyond the 12 inches creating a cantilever:  a.  7 feet to the lower level where Greenway employees were operating at the rear, left of the building.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:    On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA, at the rear left of the building; employees performing brick laying operations were exposed to fall hazards of over to 7 feet to the lower level where an employee was observed standing on a pile of block on top of the 7 foot high scaffold, to extend his reach.

On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA, at the front right of the building; employees were exposed to struck-by hazards where the working levels on the 7 foot scaffold had materials such as hand tools and bricks on them that could fall at the rear, left of the building.

Greenway Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 315842443 citation number 1, item number1 and was affirmed as a final order on 12/20/2011 with respect to a workplace located at 200 Big Mountain Road in Pelham, AL.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  The frame was used as a means of access.     On or about 10/27/2014, at 5625 Lawrenceville Highway, Lilburn, GA; employees were exposed to fall hazards of up to 7-feet where Greenway employees were observed working on scaffolding that did not have a ladder installed at the rear, left of the building.     Greenway Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 315842443, citation number 2, item number 1 and was affirmed as a final order on 12/20/2011, with respect to a workplace located at 200 Big Mountain Road in Pelham, AL.
