29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. (a) Production Area - The employer did not ensure that the work areas were kept clean and in an orderly state where employee travel was present. Pallets containing product and boxes were stored throughout the floor exposing employees to struck by and tripping hazards. (b) Warehouse - The employer did not ensure that the work areas were kept clean and in an orderly state where employee travel was present. Pallets stored against a rack were observed leaning against each other vertically and were not locked to prevent collapsing.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: (a) Spooling Department - The employer failed to ensure that guards or methods of guarding were adequate to prevent employees from coming into contact with rotating parts. The right side entry door on spooling machine #19 had the interlock system disengaged. (b) Spooling Department- The employer failed to ensure that guards or methods of guarding were adequate to prevent employees from coming into contact with rotating parts. The stripping head on spooling machine # 14 was missing a top cover.
