29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about October 29, 2014, at the job site located at 7937 Buona Sera in Springdale, Arkansas, where employees were framing a new single story residence, employees working below the employees erecting rafters were not wearing hard hats. These conditions exposed the employees to the hazard of being stuck by falling objects.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: On or about October 29, 2014, at the job site located at 7937 Buona Sera in Springdale, Arkansas, were employees were erecting rafters for a new single story residence, employees were working from a supported scaffold that extended approximately 5 feet over its supports at both ends. The platform was neither secured so the cantilevered end could support employees without tipping nor did it have guard rails to prevent employees from accessing the cantilevered ends. These conditions exposed employees to the hazard of falling approximately 16 feet to the concrete floor below.

The scaffold did not gave guard rails or nets and the employees were not using personal fall protection equipment. These conditions exposed the employees to the hazard of falling approximately 16 feet to the concrete floor below.

The employees were standing on the edge of the ceiling joists and the top plates and were not using any form of fall protection. These conditions exposed the employees to the hazard of falling approximately 10 feet to the concrete floor below.
