29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  On or about October 28, 2014, employee was exposed to falls greater than 10 feet in height to the ground level below, when working from a JLG 660SJ aerial lift with serial #0300162432, while standing on the mid-rail applying caulking to the corniced apartment wall.

29 CFR 1926.502(d)(6)(v): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any object which is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.  On or about October 28, 2014, employee(s) were exposed to falls greater than 10 feet to the ground level below, when using a tie down strap as a lanyard, which did not have a locking type snap hook, while in the JLG 660SJ aerial lift with serial #0300162432, and caulking the cornice apartment wall.
