29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. On or about October 28, 2014, four employees performing framing operations at the jobsite were not wearing hard hats, exposing themselves to overhead hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    a. On or about October 28, 2014, an employee used a 27-foot Werner extension ladder to access the top of a structure that did not extend 3 feet above the eave nor was secured from slippage, exposing the employee to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. Employees on top of a structure performing framing operations are not being protected from a 9-foot 4-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.    Pursuant to CFR 1903.9, the employer must submit documentation describing the steps it is taking to ensure compliance, including an explanation of how these steps protect its employees from falls (An abatement plan and periodic progress reports may also be required if the abatement period is more than 90 days).     The Solis Brothers Company, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 906126, citation number 02, item number 001 and was affirmed as a final order on July 8, 2013, with respect to a workplace located at 27 Deer Meadows Drive, Saint Augustine, Florida 32092.
