29 CFR 1926.403(b)(1)(i): Safety of equipment was not determined on the basis of suitability of the equipment for an identified purpose as evidenced by listing, labeling, or certification for that identified purpose.   a) On October 29, 2014 a relocatable power tap used on a construction site was not listed and labeled for use, exposing employees to the hazards associated with fire.   b) On or about October 29, 2014 a 4x4" knockout style junction box was used with a receptacle to distribute a 110 volt circuit and the junction box was not listed and labeled for use, employees to the hazards associated with fire.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:  a)  On October 29, 2014 employees using a Ridgid Cut Off saw were exposed to an electrical shock/electrocution as the path to ground was not continuous due to a missing ground pin.   b) On October 29, 2014 employees using a Milwaukee Band saw were exposed to an electrical shock/electrocution as the path to ground was not continuous due to a missing ground pin.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.405(b)(1): Unused openings in cabinets, boxes, and fittings were not effectively closed:   On or about October 29, 2014 a 4x4" knockout style junction box was used with a receptacle to distribute a 110 volt circuit.
