29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated: On October 29, 2014 an employee was not protected from a fall while operating/ working from an aerial lift, exposing the employee to a fall of approximately 12 feet. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On October 29, 2014 an employee was not protected from a fall while removing construction material from a flat commercial roof, exposing the employee to a fall of 12.6 feet above grade. RIDGELAND CONSTRUCTION GROUP INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 109000, citation number 1, item number 1b, and was affirmed as a final order on February 3, 1012, with respect to a workplace located at 133 E Ogden Ave Hinsdale, Illinois. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
