29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) At the site, on the north side of the roof, an extension ladder utilized to gain access to the roof was not erected properly. The side rails of the ladder extended 1 rung (12 inches) above the landing / access surface, exposing employees to a 10 (9.8) feet fall hazard, observed on or about 10/29/2014.
