29 CFR 1926.453(b)(2)(v):  A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  (a). In the front, street side of 12748 South Cleveland Avenue in Fort Myers, the Genie Model S-40 rough terrain aerial lift was used without fall protection to elevate employees 10 feet for access and egress from a 5-foot wide overhang along the front of the site to install Den Glass sheathing, on or about October 24, 2014.  (b). In the rear, parking lot side of 12748 South Cleveland Avenue in Fort Myers, the JLG Model 400S rough terrain aerial lift was used without fall protection to elevate employees 10 feet for access and egress from a 5-foot wide overhang along the rear of the site to install Den Glass sheathing, on or about October 24, 2014.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a). At 12748 South Cleveland Avenue in Fort Myers, employee's were installing exterior Den Glass sheathing from a 5-foot wide overhang without fall protection to protect them from a fall hazard of 10 feet, on or about October 24, 2014.
