(b). In the rear, parking lot side of 12748 South Cleveland Avenue in Fort Myers, the JLG Model 400S rough terrain aerial lift was used without fall protection to elevate employees 10 feet for access and egress from a 5-foot wide overhang along the rear of the site to install Den Glass sheathing, on or about October 24, 2014.
