29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent:  On or about January 21, 2015, the employer did not record the following workplace injury on the OSHA Form 300 or equivalent for the calendar year 2014.   a. On or about November 21, 2014, an employee injured his lower back after falling from a wagon; and was placed on restricted duty but did not lose work days.    b. On or about November 25, 2014, an employee had an infected wound and was prescribed medication, but did not lose work days.   PLEASE NOTE: CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS VIOLATION.

PLEASE NOTE: CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS VIOLATION.
