29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: Worksite located at 500 SW Grand, Oklahoma City, OK 73108. On or about October 30, 2014, an employee was observed working from an aerial lift without the use of a fall protection harness and lanyard. The employee was exposed to a fall hazard of approximately 18 feet.
