29 CFR 1926.502(d)(5): Locking-type snap-hooks were not designed and used to prevent disengagement of the snap-hook by the contact of the snap-hook keeper by the connected member: (a) Location: 181 Massachusetts Avenue, Cambridge MA, 02139 On or about 10/30/2014, the employer did not ensure that snap hooks/carabiners were installed and used as part of a personal fall protection system with proper alignment where the load would not be on the gate.
