29 CFR 1926.1053(b)(1): The side rails of portable ladders used for access did not extend at least 3 feet above the upper landing surface, and was not secured at its top to a rigid support that will not deflect and a grasping devise was not provided to assist employees mounting and dismounting the ladder.  An employee was exposed to a fall hazard while accessing a porch roof from a portable ladder.  The ladder used to access the roof did not extend three (3) feet above the upper landing surface, the top of the ladder was not secured to a rigid support and a grasping device was not provided to aid in mounting a dismounting the ladder.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards.   An employee was exposed to a fall hazard while accessing a porch roof from a portable ladder.  The ladder used to access the roof did not extend three (3) feet above the upper landing surface, the top of the ladder was not secured to a rigid support and a grasping device was not provided to aid in mounting a dismounting the ladder.  The employer did not provide training for each employee that would have enabled them to recognize the hazards and provided them with the procedures to follow to minimize these hazards.
