Abatement documentation was not received for the following violations:          Citation 2 Item 1  The Sean Cole Construction, Inc was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 968430, citation number 4, item number 1 and was affirmed as a final order on February 3, 2015, with respect to a workplace located at 4909 Blackwolf Rd, Springfield, IL.

29 CFR 1926.1053(b)(16):   Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  Employees used an extension ladder that was missing an anti-slip pad to access a roof approximately nine feet from the ground and the employer did not ensure that the damaged ladder was removed tagged and from service.

The Sean Cole Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 968430, citation number 2, item number 1 and was affirmed as a final order on April 20, 2011, with respect to a workplace located at 103 Adloff, Springfield, IL 62711.  The Sean Cole Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 968430, citation number 1, item number 1a and was affirmed as a final order on December 19, 2008, with respect to a workplace located at 512 Wake Court, Chatham, IL 62629.
