This violation occurred on or about Oct 28, 2014, on the west wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Doosan Puma 280 CNC lathe Machine, serial number PM2801152, without interlocking doors.        c.  This violation occurred on or about Oct 28, 2014, on the west wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Doosan Puma 280 CNC Lathe Machine, serial number PM2800634, without interlocking doors.      d.  This violation occurred on or about Oct 28, 2014 on the east wall of the Machine Shop where employees were exposed to struck-by and caught-by hazards when operating a Haas SLT30 Lathe Machine, serial number 3098449, without interlocking doors.      e.
