The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.     a)     At the worksite, the employer did not ensure that the extension cord that was used to link the power saw to the compressor, was equipped with a ground pin, thereby exposing employees to a potential shock hazard.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.     a)     On or about October 30, 2014, at the worksite, the employer did not ensure that employees were provided and used eye protection while using a Makita Power Saw to cut materials used to frame a new build residence, thereby exposing employees to a potential struck-by hazard.    Manuel Valasquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 966287, citation number 1, item number 1 and was affirmed as a final order on July 07, 2014, with respect to a workplace located at 294 Ravensdale Place Galloway, Oh 43119.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. Exception: When the employer can demonstrate that it is infeasible or creates a greater hazard to use these systems, the employer shall develop and implement a fall protection plan which meets the requirements of paragraph (k) of 1926.502.     a)     On or about October 30, 2014, at the worksite, the employer did not ensure that employees were provided and used fall protection while performing framing work from the front lower roof area, at heights greater than six feet, thereby exposing employees to an approximate 12 foot fall hazard.    Manuel Valasquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 966287, citation number 1, item number 2 and was affirmed as a final order on July 07, 2014, with respect to a workplace located at 294 Ravensdale Place, Galloway, Oh 43119.
