29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    (a) H.D.M. Construction Enterprises, Inc. at 3107 W. 23rd Ave., Denver, CO 80211:  On and before October 30, 2014 the employer did not ensure their employees were protected from injury, in that an employee engaged in carpentry work while on the porch roof of a residential home was exposed to falls of approximately 9 feet without the use of an adequate fall protection system.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    (a) H.D.M. Construction Enterprises, Inc. at 3107 W. 23rd Ave., Denver, CO 80211:  On and before October 30, 2014, the employer did not ensure that employees used a ladder in a manner for which it was designed in that an approximate 8 foot tall step ladder was used as an extension ladder as a means of access to the exterior of the house above the porch.  This condition exposed the employees to a fall hazard of 9 feet.
