29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation: On or about October 30, 2014, at the worksite located at 2200 Breckenridge & Colony Road in Little Rock, Arkansas, protective measures were not in place to protect employees in a trench performing sewer line/pipe repairs with an accumulation of water in the bottom of the trench. This condition exposed employees to the hazard of potential cave-ins.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): On or about October 30, 2014, at the worksite located at 2200 Breckenridge & Colony Road in Little Rock, Arkansas, no protection was provided on the north and south ends of the vertical-walled trench that employees worked in to perform sewer line/pipe repairs. This condition exposed employees to the hazard of cave-ins. PM Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 314301318, citation number 1, item number 1 and was affirmed as a final order on May 4, 2012, with respect to a workplace located at 235 Cedarhurst Drive in San Antonio, Texas.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: On or about October 30, 2014, at the worksite located at 2200 Breckenridge & Colony Road in Little Rock, Arkansas, the shoring shield sheeting on the west side of the excavation that employees worked in to perform sewer line/pipe repairs did not extend above the top of the trench. This condition exposed employees to being struck by rolling excavated materials.
