Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.150(c)(1)(iv): There was not at least one fire extinguisher located adjacent to stairways on each floor of a multistory building:  a) First Floor - A laborer was observed cutting steel rebar with a Stihl gasoline powered chop saw and there was no portable fire extinguisher available in the event of a fire, on or about 10/16/2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Back Porch - A laborer was observed standing at the edge of a concrete deck while cutting steel rebar with a Stihl chop saw, model number TS420, and exposed to a 10 ft. fall hazard, on or about 10/16/2014.  b) First Floor East Wall - Laborers were observed walking near open floor holes measuring between 4 ft., and 2 ft., wide and exposed to a 10 ft. fall, on or about 10/16/2014.  c) First Floor at Temporary Service Panel - Laborers and foreman were observed near a floor hole that measured 22 inches by 48 inches and exposed to a 10 ft. fall hazard, on or about 10/16/2014.
