29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: a) Third Floor at NW Lower Roof Section Hatchway - An employee was not provided refresher training in recognizing hazards associated with ladder usage as evidenced by noted hazards, such as but not limited to; employees using a Warner portable extension ladder that was separated from the back section, had a rubber slip resistant pad missing from a foot, and the horizontal distance from the top support to the foot of the ladder was less than one-quarter of the working length of the ladder, on or about 10/29/2014. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
