The employer failed to provide certification of abatement on each of the cited items: a) Lock Out/Tag Out procedures for controlling the hazardous energy on the injection molding machines. b) Annual Certification of periodic inspections of the energy control procedures that are performed. c) Adequate completion of the OSHA Form 300. d) Creation, implementation and maintenance of a hazard assessment for the facility. e) Hazard Communication training on safety data sheets and the labeling system, for the employees.
