29 CFR 1910.138(a):  Employers did not select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes.   This violation was observed on or about October 29, 2014, in the press area, the employer did not ensure employees used cut resistant gloves when trimming plastic and/or protective gloves when handling or exposed to heated objects or machinery exposing employees to struck-by and thermal hazards.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.   This violation occurred on or about October 29, 2014, when authorized employees required to perform operations together which require lockout/tagout such as, but not limited to, changing out molds, all involved authorized employees were not required to  affix their locks to the main disconnect or a group lockout device exposing employees to the hazards of being caught-in the machinery.

Case No. 2  Date of injury 02/19/14  the description field (Column F) did not indicate what caused the twisted knee.   Case No. 6  Date of injury 06/14/14  the description field (Column F) did not indicate what caused the smashed left thumb).
