The floor surface of a spray booth and operator's working area, if combustible, shall be covered with noncombustible material of such character as to facilitate the safe cleaning and removal of residues.  a)    The floor of spray booth #1 in the painting area of plant was covered with cardboard on 11/6/2014 during the inspection and did not have a noncombustible material over the top of it.  b)    The floor of spray booth #2 in the painting area of the plant was covered with cardboard on 11/6/2014 during the inspection and did not have a noncombustible material over the top of it.

29 CFR 1910.107(b)(5)(i): The spraying operations (except electrostatic spraying operations) were not designed, installed and maintained such that the average air velocity over the open face of the booth (or booth cross section during spraying operations) was not less than 100 linear feet per minute. Electrostatic spraying were conducted when the air velocity over the open face of the booth was less than 60 linear feet per minute (or more, depending on the volume of the finishing material being applied and its flammability and explosion characteristics). Visible gauges or audible alarm or pressure activated devices were not installed to indicate or insure that the required air velocity was maintained. Filter rolls were not inspected to insure proper replacement of filter media.   a)    Spray booth #1 in the painting area of plant was equipped with a JBI Inc. gauge to monitor air velocity on 11/6/2014 during the inspection and it was not operational. Air flow measured during inspection did not average 100 linear feet per minute.  b)    Spray booth #2 in the painting area of the plant was equipped with a BINKS gauge to monitor air velocity on 11/6/2014 during the inspection and it was not clear whether it was operational.  c)    Spray booth in the cab finishing department was equipped with a DWYER MARK II gauge to monitor air velocity on 11/6/2014 during the inspection and it was not operational.

29 CFR 1910.134(h)(3): 29 CFR 1910.134(h)(3)(ii)[A]: The employer did not ensure that respirator inspections included a check of respirator function,  tightness of connections,  and the condition of the various parts including,  but not limited to, the face piece,  head straps,  valves,  connecting tube,  and cartridges, canisters or filters:  a)    The company was not monitoring the employees use of respirators and the respirators condition for spray finishing at the plant at the time of the inspection on 11/6/2014. A SURVIVAIR respirator was found to be missing one of its valves during the inspection. Annual training and evaluations of the program were not being performed.
