On or about 23 October, 2014; employees of Jose Cardona dba Cardonas Roofing were performing residential roofing activities without conventional fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Cardona's Roofing specifically observed at the jobsite 3809 N. 17th Street, Omaha, NE: The employer is failing to provide training for employees potentially exposed to fall hazards. On or about 23, October 2014 employees of the business were observed working at heights greater than 6 feet without fall protection. The employer had not provided the employees training to educated them on the requirements to use fall protection and the different types of fall protection that are available for their use.
