29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). On or about October 22, 2014, on the east side of the construction site, employees engaged in framing activities without fall protection were exposed to a fall hazard of approximately 12 feet to the lower level.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided. On or about October 22, 2014, on the east side of the construction site, the employer did not provide a ladder where there was a break in elevation for employees to safely access and egress the upper landing and building structures exposing the employees to an unprotected fall hazard of more than 12 feet.
