29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a. Site- Employees exposed to electric shock and arc flash hazards were not provided with safe electrical work practice training including lock out/tag out procedures, required personal protective equipment, and voltage testing procedures from the employer. On or about October 27, 2014 a temporary employee who was not provided with safe electrical work practice training sustained an electric shock and fell off of a step ladder when they contacted live power operating at 277 volts.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a. Site- An employee was not protected from electric shock where a lighting circuit that was being worked on was not de-energized and locked out. On or about October 27, 2014 a temporary employee who was working on a live lighting circuit sustained an electric shock and fell off of a step ladder when they contacted live power operating at 277 volts.
