On or about October 30, 2014, employees of a subcontractor, Gladstone, Inc. were working from a 2-buck high scaffold system that was not fully planked on each working level, exposing the employees to fall hazards up to 13 feet. b. On or about October 30, 2014, the superintendent climbed the 2-buck high scaffold system to show the employees of Gladstone, Inc. something about the top two windows. The scaffold was not fully planked on each working level. He exposed himself to fall hazards up to 13 feet.

On or about October 30, 2014, the superintendent for Scherer Construction climbed a two-buck high scaffold system and exposed himself up to a 13 feet fall hazard because there was no guardrail system, safety net system, personal fall arrest system or any other type fall protection method being used.
