29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D):  A.) Elkhorn / Keystone Natural Gas Processing Plant, LLC, 1990 Zimmerman Hill Road, Clarendon, PA 16313: On or about October 30, 2014, and at times prior thereto, employee periodically conducted servicing, maintenance and repairs to the Ajax compressor.  The Lockout/Tagout procedures for the Ajax compressor were not specific in that they did not contain information including, but not limited to:  1.) the scope of the procedure (identity of the equipment),  2.) specific identify of energy sources and their magnitudes,  3.) specific identification of isolation points/locations for each energy source,  4.) specific sequential techniques for bringing the equipment to a zero energy state,  5.) specific methods/techniques to dissipate stored energy,  6.) specific techniques for verifying the equipment is in a zero energy state, and  7.) specific methods for returning the equipment to service following the completion of the servicing, maintenance or repairs.    B.) Elkhorn / Keystone Natural Gas Processing Plant, LLC, 1990 Zimmerman Hill Road, Clarendon, PA 16313: On or about October 30, 2014, and at times prior thereto, employee periodically conducted servicing, maintenance and repairs to the plant air compressor, and plant process pumps.  Specific Lockout/Tagout procedures for the compressor and pumps had not been developed.
