29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood whcih completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut.  Location; 145 west 126th Street, New York, NY 2nd Floor  On or About October 30, 2014  a). Employees were using a portable 10" table saw to rip materials to repair/restore old entry doors and were exposed to amputation hazards because the saw blade above the rip table was not protected by the manufacturer provided guard.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous  LOCATION: 145 126th Street, New York, NY 10035, second floor front.  On or About October 30, 2014  a) Employees were utilizing flexible extension cords to energize portable DeWalt Brand slide saw and table saw. The saws were not protected by a continuous path to ground because the flexible cord was missing the ground pin.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such hole.  Location: 145 West 126th Street, New York, NY Roof level, Second Floor, 1st Floor.  On or About October 30, 2014.  A). Employees were engaged in the removal and demolition of roofing materials at the above location and were exposed to falls over 6 feet to the floor below because employer did not ensure each employee was protected by personal fall arrest systems, covers, or guardrail systems erected around such hole.

29 CFR 1926.1052(c)(12): Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:  Location 145 West 126th Street, New York, NY , first and second floor.  a). Employees engaged in a GUT/REHAB project were not protected from falling to the floors below because the handrail systems around the existing stairwells had been demolished.  Employees were utilizing these stairs to traverse throughout the project.
