29 CFR 1910.119(f)(3): The operating procedures were not reviewed as often as necessary to assure that they reflect current operating practice, including changes that result from changes in process chemicals, technology, and equipment, and changes to facilities: a. Area One, Platformer Heater - The employer did not clearly state whether the written Critical Instrument Test Procedure Number RG/CIXSD13603KV entitled: "Platformer #2 Reheater (13-H-3) Emergency Fuel Gas Shut-off System (XSD-13603) No Testing of High/Low Pressure Transmitters" was for the quick test or for the long test. On or About October 31, 2014 b. Area Two, VGO Heater -The employer did not ensure that written Critical Instrument Test Procedure Number RG/CIXD32600KV entitled: "VGO Heater Emergency Fuel Gas Shut-off System, (XSD-32600) (No Testing of Low Flow or Low/High Pressure Transmitters)" was updated to reflect that the employer had converted VGO Heater (Vacuum Gas-Oil Heater) from gas-oil service to diesel service. On or About October 31, 2014
