29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: a) Eleuterio Islas-Baltasar @ 9071 E. Colorado Drive in Denver, CO 80203: On and before October 17, 2014 the employer did not ensure employees were protected from fall hazards in that employees conducting roofing operations on a 10 in 12 pitched roof utilized the front attachment of their harnesses rather than the back D-ring. In the event of a fall this condition exposed employees to physical (i.e. facial) trauma from improper use of fall protection.
