29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) (a) On October 31, 2014, at jobsite located on the NE corner of E 3rd St and Holmes ST, Kansas City, MO., employees were exposed to serious fall hazards, in that the working level scaffold platform that was not fully decked across the scaffold uprights. As a result, the employees were exposed to a 12 feet - 3 inch fall to the concrete surface below. Abatement certification for this violation is not required.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used: (a) On October 31, 2014, at jobsite located on the NE corner of E 3rd St and Holmes ST, Kansas City, MO., employees were exposed to serious fall hazards, in that they were not provided with a safe means of access to the top level scaffold platform which caused employees to climb up and down the scaffold frame. As a result, the employees were exposed up to an approx. 12 feet - 3 inch fall to the concrete surface below. Abatement certification for this violation is not required.

As a result, the employees were exposed to a 12 feet - 3 inch fall to the concrete surface below. Abatement certification for this violation is not required.
