29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    A competent person did not conduct suitable inspections of the jobsite and equipment when employees were performing commercial construction in that:  Employees were working on a roof at a height of 17 foot using improperly installed fall protection equipment.

29 CFR 1926.502(d)(6)(iv): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself.     Employees were utilizing lanyards whose snaphooks were connected directly to horizontal lifeline rope while performing roofing operations at a height of seventeen (17) feet above ground.

29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two:     The employer did not ensure the horizontal lifeline was designed, installed and used under the supervision of a qualified person when:  employees were exposed to the hazard of falling 17 feet due to improper design and installation of lifeline.
