29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about October 30, 2014, at the jobsite, employees used a Hitachi drill that was supplied power by a yellow extension cord that was severly cut and frayed exposing employees to electrical shock hazards.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system. On or about October 30, 2014, employees performing decking activities on the roof of a residential framed structure were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.
