29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) Jobsite 115-117 South Pittsburgh Street, Connellsville, PA: On or about October 31, 2014, two employees were working in the bucket of a CAT TH103 approximately 12 feet high without the use of fall protection.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).  (a) Jobsite 115-117 South Pittsburgh Street, Connellsville, PA: On or about October 31, 2014, employees were working from a basket of an extensible boom to complete demolition work of the building and was not properly tied off.
