29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): (a) Jobsite: On or about October 29, 2014, operators of the JLG lull were not certified, trained and evaluated to operate the lull. One employee was operating the JLG lull to lift concrete form panels into place.
