29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    a) On or about September 22, 2014, while cleaning the number three slug press located in the Consumable division at 22801 St. Clair Ave., Cleveland, Ohio 44117, the press operators were permitted to clean the ram, cylinder, and bin/feed chute, exposing them to the point of operation, while the press is still energized.  An energy control procedure does exists for maintenance employees but is not utilized by press operators during cleaning operations.    b) On and before October 31, 2014, while cleaning the number one slug press located in the Consumable division at 22801 St. Clair Ave., Cleveland, Ohio 44117, the press operators were permitted to clean the ram, cylinder, and bin/feed chute, exposing them to the point of operation, while the press is still energized.  An energy control procedure does exists for maintenance employees but is not utilized by press operators during cleaning operations.    c) On and before October 31, 2014, while cleaning the number two slug press located in the Consumable division at 22801 St. Clair Ave., Cleveland, Ohio 44117, the press operators were permitted to clean the ram, cylinder, and bin/feed chute, exposing them to the point of operation, while the press is still energized.
