29 CFR 1926.1413(a)(2)(i)(D): A competent person did not inspect wire rope for apparent deficiencies such as: Improperly applied end connections. On or about October 30, 2014 the dead end of the load line was clamped to the live end just above the wedge and socket fitting located at each end of the wire rope. The wire rope was located on a D6D Cat, Unit number 101-149 found at the gas pipeline construction site.
