29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems: A.) Holko Enerco Inc.; Construction site, Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14, employees were working on an unprotected roof without the use of fall protection. The section of the roof personnel were working on was not protected by a guardrail system or equivalent parapet. The condition exposed employees to a fall hazards of up to 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: A.) Holko Enerco, Inc.; construction site; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14, employees were utilizing a ladder to gain access to a roof. The ladder was extended just over 2 feet above the roof line/are of the roof where personnel would mount/dismount the ladder. The condition exposed personnel to a fall hazard of up to 20 foot potential fall.
