29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    (a)  NEAR BACK WAREHOUSE ENTRANCE - The employer did not maintain a minimum of thirty-six (36) inches of clearance in front of the electrical panels next to the back warehouse entry way, thereby exposing employees to possible injury, on or about October 30, 2014.    (b)  UPSTAIRS PRODUCTION ROOM NEAR BREAK AREA - The employer did not maintain a minimum of thirty-six (36) inches of clearance in front of the electrical panels next to the employee break area, thereby exposing employees to possible injury, on or about October 30, 2014.    No abatement certification or documentation required.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:    (a)  FACILITY WIDE - Employees used products including, but not limited to Glass Glo Concentrate, Bowl Bryte, Germicidal Bleach, and Valient Acid Bowl Cleaner, and the employer had not developed and implemented a written hazard communication program, thereby exposing employees to serious injury, on or about October 30, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
