OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to stuck-by hazards:    a)  676 Halla-Bama Drive, Shorter AL - Production floor; On or about October 28, 2014 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to walk and work around steel storage racks that had damaged columns and support braces.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven -7 feet or less above floors or platforms were not enclosed:    a)  Final station crimp line 1; On or about October 28, 2014 and at times prior thereto the employer exposed employees to amputation hazards in that employees were allowed to walk and work within 16 inches of an unguarded rotating exposed chain and sprocket that were within four feet of the floor.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:     a)  Compressor Room; On or about October 28, 2014 and at times prior thereto the employer exposed employees to fire hazards in that employees were allowed to walk and work in the same area where oxygen cylinders and fuel gas (acetylene/propane) cylinders were stored together without a barrier as required by the standard.
