29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: LOCATION: Main Plant and Switch Gear Buildings: Instance A) A Hercules B6-44 telescoping boom attachment, was being used on the forks of the Clark GCS 25 MB LP forklift to move loads of 12' x 1/4" copper buss bar. No written approval from the forklift manufacturer was obtained by the employer. The addition of the boom attachment decreases the capacity of the forklift. Instance B) A Hercules B6-44 telescoping boom attachment, was being used on the forks of a CAT T80C LP forklift to move loads of 12' x 1/4" copper buss bar. No written approval from the forklift manufacturer was obtained by the employer. The addition of the boom attachment decreases the capacity of the forklift.
