Job Site - On or about 11-3-14 - employees were installing asphalt shingles on a three on twelve pitch roof exposed to a nineteen foot fall hazard with no fall protection systems in use.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupers for personal protection. a) Applegate Apartments, Plainville, CT. Job Site - On or about 11-3-14 - Employees were using a electric powered Campbell Hausfeld Compressor for pneumatic staple guns to install shingles that was not equipped with a GFCI in the system. Compressor was drawing power from an extension cord plugged into a 15 amp receptacle inside the building.
