29 CFR 1926.451(d)(3)(x): Tiebacks shall be installed perpendicular to the face of the building or structure, or opposing angle tiebacks shall be installed. Single tiebacks installed at an angle are prohibited:    a) On or about October 29, 2014 - at a jobsite located at 1457 E. 54th Place; Chicago, IL. - an employee was performing tuck pointing operation from a two point suspended scaffold that did not have the tiebacks installed perpendicular to the structure, thereby exposing the employees to falls hazards of approximately thirty (30) feet.

29 CFR 1926.451(d)(12)(i): There shall be a minimum of 3 wire rope clips installed, with the clips a minimum of 6 rope diameters apart:  a) On or about October 29, 2014 - at a jobsite located at 1457 E. 54th Place; Chicago, IL. - an employee performing tuck pointing operations from a suspended scaffold that was erected without using three wire rope clips, thereby exposing the employee to falls of approximately thirty (30) feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access:    a) On or about October 29, 2014 - at a jobsite located at 1457 E. 54th Place; Chicago, IL. - an employee performing tuck  pointing operations from a suspended scaffold was not provided with an access ladder or equivalent safe access, thereby exposing the employee to falls hazards of approximately thirty (30) feet.

29 CFR 1926.451(g)(3)(i): When vertical lifelines are used, they shall be fastened to a fixed safe point of anchorage, shall be independent of the scaffold, and shall be protected from sharp edges and abrasion. Safe points of anchorage include structural members of buildings, but do not include standpipes, vents, other piping systems, electrical conduit, outrigger beams, or counterweights:    a) On or about October 29, 2014 - at a jobsite located at 1457 E. 54th Place; Chicago, IL. - an employee performing tuck pointing operation from a suspended scaffold was not using an independent anchor point for the employees lifeline. The employee was thereby exposed to falls hazards of approximately thirty (30) feet.
