Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   a) Transfer Truck Loading Area- On or about November 03, 2014, employees who were working while standing underneath a cotton seed hopper where the material was released into a dump trailer were exposed to engulfment hazards.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: the employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Transfer Truck Loading Area- On or about November 03, 2014, employees who were working while standing on steel beams adjacent to a cotton seed hopper were exposed to fall hazards of about 13.5 feet onto the ground.    b) By module feeder - On or about November 03, 2014, the stairway leading to the up-stairs office did not have a mid-rail in place, exposing employees to fall hazards of up to 15.0 feet onto the concrete floor.    c) At the facility - On or about November 03, 2014, the guardrail system surrounding the stairway leading to the pit/basement area did not have a mid-rail in place, exposing employees to fall hazards of about 8 feet onto the concrete floor.     d) At the facility - On or about November 03, 2014, numerous platforms within the cotton ginning equipment were missing mid-rails, exposing employees to fall hazards from 8 - 25 feet onto the concrete floor.

29 CFR 1928.57(d)(1)(iv): 29 CFR 1928.57(d)(1)(iv): Pulleys of V-belt drives were not completely enclosed or guarded by location whether or not railings are present. The open end of the pulley guard shall be not less than 4 inches from the periphery of the pulleys.  a) At the facility, on or about and at times prior to November 3, 2014, the employer allowed operators and others to work near the unguarded belt and pulley of the burner/fan, exposing employees to entanglement and amputation hazards.
