29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.  Along the north end of the excavation, on or about November 03, 2014, employees were exposed to the hazards of struck-by when a Caterpillar excavator model 320D was being operated while located beyond the edge of the excavation.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladder(s) were not used at an angle such that the horizontal distance from the top support to the foot of the ladder was approximately one-quarter of the working length of the ladder.  Along the south end of excavation, on or about November 03, 2014, an employee was exposed to the hazards of falls while accessing an excavation approximately 14' deep by 6' wide by 20' long using an extension ladder that was positioned parallel up against the inner wall of a trench box and did not provide proper footing.

29 CFR 1926.1053(b)(6): Ladder(s) was not used only on stable and level surface(s) unless secured to prevent accidental displacement:  Along the south end of the excavation, on or about November 03, 2014, an employee was exposed to the hazard of falls when using an extension ladder as access into an excavation and was placed along the south end of a protective excavation box in which the ladder was not positioned firmly on the ground surface and was tied with a strap onto the top of the box.
