29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about October 17, 2014, on the campus of Oklahoma Baptist University, employees were allowed to work on a steep pitched roof without fall protection, exposing the employee to the hazard of falls of approximately 32 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about October 17, 2014, on the campus of Oklahoma Baptist University, an employee was allowed to work on a steep pitched roof without being trained to recognize and protect himself from the hazard of falls. The employee was exposed to the hazard of falls of approximately 32 feet.
