29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about November 4, 2014 the Republic Lagun lathe was not equipped with a chip shield or chuck guard to prevent flying chips from striking the operator and to protect the operator from inadvertent contact with rotating chuck.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) On and at times prior to November 4, 2014 the distance between the abrasive wheel periphery and the end of the guard member exceeded one-fourth inch on the Delta bench grinder near the surface grinder.    b) On and at times prior to November 4, 2014 no tongue guard were installed on the bench grinder near the weld station.   c) On and at times prior to November 4, 2014 the tongue guard on the Delta 6 inch bench grinder was not adjusted to within one-fourth of an inch of the wheel periphery.   d) On and at times prior to November 4, 2014 the two tongue guards on the bench grinder near the CNC machines were not adjusted to within one-fourth of an inch of the wheel periphery.
