29 CFR 1926.601(b)(4): No employer shall use any motor vehicle equipment having an obstructed view to the rear:       a) 18627 Robinsonville Road, Lewes, DE -  Employer permitted the use of motor vehicle equipment with an obstructed view to the rear.  Employees conducting concrete form work at a residential construction site operated a 2013 Chevy Silverado 3500 Heavy Duty dually work truck, DE license plate #C344519, with an obstructed view to the rear.  The truck did not have a reverse signal alarm, and an observer did not signal it was safe to back up.  On October 31, 2014, an employee was fatally struck-by and run over by the work truck.  The violation was documented on or about November 1, 2014.
